For many years, Hartgrove Behavioral Hospital has been the focus of serious concerns from regulators, advocates, and former patients. Multiple investigations have described the facility as unsafe and poorly supervised, with issues including inadequate staffing, chaotic unit conditions, questionable medication practices involving minors, communication failures with guardians, and repeated incidents of patient-to-patient violence. Reviews also found that Hartgrove had not consistently addressed environmental hazards or implemented sufficient protections for vulnerable youth, creating a setting where physical, emotional, and sexual abuse could occur without proper intervention.

 

The Allendale Association, which runs group homes and youth residential treatment facilities in Lake Villa, Waukegan, and nearby areas, has faced serious allegations of abuse, neglect, and deadly misuse of restraint over the years. Over recent years, multiple instances of violence and abuse have occurred in the facilities. These events have spurred criminal prosecutions, civil lawsuits, and regulatory/oversight reviews, as well as investigations by the state welfare agency though survivors, advocates, and community members continue to raise concerns about systemic failures, oversight deficits, and whether the facility has reformed sufficiently to prevent further abuse.

 

Rock River Academy was a residential treatment center for girls in Rockford, Illinois.  Many former residents have alleged pervasive sexual, physical, and emotional abuse during their time there. 

According to lawsuits and investigations, staff allegedly coerced minors, used psychotropic drugs and other substances to keep residents compliant, and allowed or ignored other residents sexually abusing each other. The facility reportedly generated hundreds of police reports over several years for SA-related issues among its female residents. RRA  shut down in 2016 and multiple  lawsuits have been filed against its operator (Universal Health Services) alleging systemic negligence: failure to screen staff properly, failure to protect vulnerable youth, failure to investigate or report abuse, and allowing a culture of exploitation to persist.
